Academic Action Form: 
Download form
This form is used to get approval for transfer credit; prior approval from the dean's office is always required for transfer coursework.
Transfer of Major Coursework Form: Download form
This form must accompany the Academic Action Form if you need approval to take any major coursework at another institution.
Request for Degree Plan Form: Download form
Students must submit this form to request an 'official' degree plan when they have completed 54 hours or at the end of their sophomore year.
Intent to Graduate Form: Download form
Students must submit the Intent to Graduate form to the College of Fine Arts office when they have completed 96 hours of coursework or 2 semesters to graduate, whichever comes first. (Students must have a degree plan on file before the Intent to Graduate will be completed.)
